If you are experiencing a mental health crisis, we urge you to reach out to the 988 Lifeline, a 24/7 confidential crisis line offering assistance to individuals experiencing a suicidal crisis or emotional distress.

Dona Ana Connect is a system powered by Unite Us that operates as a closed-loop software system, giving individuals the option to self-refer or connect with    Community Health Workers (CHWs).  With this proactive approach, we can help residents of Doña Ana County access essential health and social services.

Our goal is to enhance the well-being of Doña Ana County residents by providing access to health and social services:

  1. Connect residents to health and social services provided by our partnering agencies.
  2. Prevent individuals from being overlooked in organizational systems
  3. Monitor and ensure that referrals are consistently tracked until services are provided.
  4. Strengthen collaboration with current partners
  5. Establish relationships with new agencies to enhance coordination for comprehensive care.

 

Budget: CONNECT Doña Ana is paid for by a combination of County and Grant Funds.
